Patients often require a combination of medical and rehabilitative treatment, following surgery, an accident or illness. These patients no longer require care in an acute care setting but still need medical care that is more intensive than that found in the home. Remington Transitional Care of Richardson was designed to fill this treatment gap. We treat those patients that have the potential to improve quickly and be discharged home or to a less intensive setting. Patients who have been stabilized in an acute care hospital are excellent candidates for post-acute care at Remington Transitional Care of Richardson where they receive the continued medical and therapeutic management they require to achieve maximum recovery.
Individualized care and/or rehabilitation plans are designed for each patient and may include:

- Extensive I.V. Care and Management
