Patients often require a combination of medical and rehabilitative treatment, following surgery, an accident or illness. These patients no longer require care in an acute care setting but still need medical care that is more intensive than that found in the home. The Transitional Care Unit at Mission Valley was designed to fill this treatment gap. Patients at the TCU have the potential to improve quickly and be discharged home or to a less intensive setting. Patients who have been stabilized in an acute care hospital are excellent candidates for post-acute care at the TCU where they receive the continued medical and therapeutic management they require to achieve maximum recovery.

Individualized care and/or rehabilitation plans are designed for each patient and may include:

  • Respiratory Therapy
  • Post-Surgical Care
  • Extensive I.V. Care
  • Palliative Care
  • Wound Care
  • Joint Replacement Rehabilitation
  • Physical, Occupational & Speech Therapies

Amenities will include:

  • Flat Screen TVs
  • WiFi access
  • Bistro/ Internet Café
  • ADL Kitchen
  • ADL Bathroom
  • Speech Therapy Room
  • 3 dining rooms with serving kitchens
  • Resident selected dining times
  • 4 living rooms for resident & family use
  • 2 large, secure outdoor courtyards
